FOXO4-DRI: is the senolytic promise holding up 8 years on?

FOXO4-DRI made a big splash with the original mouse work showing selective clearance of senescent cells and some striking functional restoration (fur density, renal function, fitness in aged mice). It's been roughly eight years, so I wanted to ask: what's actually happened since?

My read is that the concept — disrupting the FOXO4-p53 interaction to push senescent cells into apoptosis — is still cited as an elegant proof of principle for targeted senolytics. But I haven't found much in the way of robust independent replication or any meaningful human clinical development for this specific peptide. The broader senolytic field has moved a lot (dasatinib+quercetin, various small molecules) while this particular D-retro-inverso peptide seems to have stayed mostly at the original-lab stage.

Am I missing later work? I'd genuinely like to be corrected. It'd be a shame if such a clean idea just stalled.

Your read tracks mine. It's one of the most-cited 'wow' results in senolytics and simultaneously one of the least-replicated at the level of independent labs reproducing the whole in vivo phenotype. Proof of principle, not a mature program.

The senolytic field generally is dealing with the 'which senescent cells, in which tissue, at what time' problem — senescence isn't monolithic, and clearing it isn't uniformly beneficial. So even a working senolytic peptide raises hard questions about specificity and timing that the original splashy result doesn't settle.

One practical wrinkle people gloss over: DRI peptides are a clever design for stability, but delivery, biodistribution and manufacturing at any scale are non-trivial, which is part of why small molecules have carried more of the clinical momentum.

No comment on use of any of this. Just noting that 'elegant mechanism in mice' to 'viable intervention' is a very long road, and for this compound the road looks mostly untravelled since the original papers.
