Cerebrolysin gets talked about as a neuroprotective / neurotrophic agent, and there genuinely are clinical trials in stroke and dementia contexts — it's not vaporware. But when I dig into the meta-analyses the picture is muddier than the enthusiasm suggests: some show modest signals, some are underpowered, and there are questions about heterogeneity and publication patterns.

Has anyone here actually read through the Cochrane-type reviews on it? I'd love a sober take on where the evidence genuinely sits for the stroke and Alzheimer's indications, separate from the general-purpose "neuroprotection" halo it gets in forums.

The Cochrane reviews are the right place to anchor. My honest summary: for acute ischemic stroke, reviews have generally not found convincing evidence of benefit on death/dependency, and flagged concerns about the evidence base. For vascular dementia and Alzheimer's there are some positive-looking trials but again heterogeneity and quality caveats.

So it's a substance with real trials and a genuinely interesting rationale (it's a porcine-brain-derived peptide mixture, not a single molecule), but "proven neuroprotectant" overstates it. It's a candidate, studied in disease populations, not a validated cognitive booster for healthy people. Big distinction people gloss over.

That matches my read, thanks for confirming I wasn't missing a landmark trial. The disease-population vs healthy-person gap is the thing I wish got stated in every one of these threads. A signal in vascular dementia tells you almost nothing about a 30-year-old wanting sharper focus.

Worth flagging that it being a biological mixture (not a defined synthetic peptide) also makes batch-to-batch consistency and characterization a real scientific question, independent of efficacy. That alone makes me treat forum anecdotes about it cautiously. For anything real, a clinician who can weigh the actual trial data for a specific situation is the move.
